Causes of Breast Cancer:

A number of external factors have been considered as contributory including diet, obesity, viruses and use of alcohol. Environmental factors such as chemicals and radiation also plays a role.

Risk Factors:

Women are at greater risk for breast cancer since 99% occurs in women. Increasing age also increases the risk of developing cancer. It is rare in women under 25 years of age and predominates in women over the age of 60. Positive family history is an important risk factor especially if the involved member with breast cancer was premenopausal, had bilateral disease and was a first degree relative(mother, sister or daughter). The role of family history however is often overestimated.

Clinical Manifestations:

Breast cancer is usually first detected as a single lump in the breast. It occurs most often in the upper outer quadrant of the breast. The rate at which the lesion grows varies considerably. Slow growing lesions are often associated with a lower mortality rate. On Palpation the breast cancer is characteristically hard, irregularly shaped, poorly delineated and non-mobile. It is painless and non-tender in 90% of the cases.

Only 5% to 10% of breast cancers present as nipple discharge. The discharge is usually unilateral and may be clear or bloody. Nipple retraction may occur. Often there is edema of the skin over the breast giving it a characteristic appearance like that of an orange peel. In later stages infiltration, indurations and dimpling of the overlying skin may occur.

Management or Treatment Plan:

The treatment or the management options are classified in two called as Diagnostic and Therapeutic.

Diagnostic:

The following are done for the patient once when they come to the clinic with the positive signs pr symptoms of what has explained earlier.

  • History of risk factors
  • Physical examination of breast and lymphatics
  • Mammography
  • Biopsy
  • Estrogen receptor assay

Therapeutic:

Surgery:

  • Lumpectomy
  • Simple mastectomy
  • Modified radical mastectomy
  • Radical mastectomy
  • Extended radical mastectomy

Radiation therapy:

  • Primary radiotherapy
  • Adjuvant radiotherapy
  • Palliative radiotherapy

Chemotherapy:

  • Adjuvant chemotherapy
  • Chemotherapy of recurrent disease

Hormonal therapy:

  • Hormones (Eg: Tamoxifen, aminoglutethimide)
  • Surgical hormonal therapy
